In preparation for implementing enhancements to UCPath e-Forms, all e-Forms will be unavailable from Wednesday, May 27, 2026, through Sunday, May 31, 2026, including:
- Accruals and Service Adjustment Request
- Request to Exceed Maximum Vacation Accrual Limit
- State Agency Prior Service Verification
- Out of State Income Tax Withholding
- Foreign Source Income Statement
- Records and Data Request
- Benefits Request
- Update Dependent Information
UCPath will process all previously submitted forms during this downtime.
All e-Forms will be available again on Monday, June 1, 2026.
Action Item: Please ensure that you make any necessary e-Form updates before these dates.

Updates: Users will notice an updated look and improved accessibility, but the underlying e-Form functionality will remain the same.
